抹式兼蜘明細作成指示照台票

My friend sent it to me saying that his girlfriend's mom typed it to him, and I study Japanese so he thought I was the next best thing to somebody who actually knows some form of Chinese.

Any help would be greatly appreciated, we've tried everything from google translate to breaking down the sentence on nciku and haven't been able to yield anything coherent.

It appears to have structure, but doesn't make any sense to me either...
